We all know that forms and paperwork are an important part of today’s healthcare environment. We have worked hard to make the process as easy as possible for all of our patients. At Southlake Dermatology, all patient intake is done electronically (saving time and trees)! You can even pay your co-pay and/or any outstanding balance from your own personal device.

Payment is expected at the time services are rendered or products are purchased.
Payment Options
For your convenience, we accept many insurance plans and offer an array of payment options including: Cash, checks, most major credit cards including Mastercard, VISA, and Discover.
Before Your Visit
Prior to your visit, you will receive an appointment confirmation text or email from us. If this is your first visit, you will also be prompted to complete your new patient intake forms. Clicking the link to confirm your appointment will automatically lead you to complete your patient registration and prompt you through the entire process. It’s easy to fill out, and we will remember your answers for your next visit, so you only have to make necessary changes in the future.
Registering before your visit helps us serve you better and faster when you come into the office.
Rewards Program
Benefits of Using Our Shop
Register for an account on our store and you’ll automatically be enrolled in our Rewards Program.
We appreciate every patient that utilizes our online store, and to show our appreciation we reward you for every purchase and engagement. Compare our program to any other rewards program out there, and you’ll see that we offer you more savings and more flexibility!
Each point you earn is worth $1 to spend on-line.
How You Earn Points
- By registering for an account on our store, you will automatically be enrolled in our Rewards Program and will start to earn points. Earn 1 point just for signing up!
- Earn 2% back in reward points for every store purchase. That is, for each on-line purchase, earn 1 point for every $50 of your subtotal.
- Earn 1 point for every product review, up to 2 reviews per day.
- Earn 2 points for signing up for our newsletter/blog.
- Earn points for referring your friends to our store. For each friend that purchase on the store, your friend will get a 10% discount on their first purchase. You will earn $10 in reward points for each friend you refer.
More great benefits
- Points are awarded as soon as your order ships.
- The sky is the limit! There is no maximum number of points you can earn per purchase or in total.
- You can start using points as soon as you’ve earned your first point.
- You may use as few or as many points as you would like for each purchase.
- Points may be used on any item in our online store.
Other rules that apply
- Points expire 1 year from the date they are earned.
- Points may only be used towards on-line purchases; they may not be used for in-office purchases.
- If you checkout as a Guest instead of a registered user, you forfeit the opportunity to earn rewards points from that purchase.
- Points are not issued on gift card purchases or redemptions, sales tax, or shipping charges.
- If you return an order, the corresponding points will be deducted from your account.
- Points have no cash value and may not be redeemed for cash.
- Points are non-transferrable.
- Points from multiple accounts may not be combined.
- We reserve the right to change the rules of the Rewards Program at any time.
